Smoking before bed: what it does to your sleep.

Most smokers know a cigarette is a stimulant. Fewer know that the one they smoke last has an outsized effect on the night that follows. This guide walks through what sleep-lab studies actually measured, in plain language, and where the evidence is thinner than the headlines suggest.
What the studies actually measure
The strongest sleep evidence comes from polysomnography: overnight recordings of brain activity, eye movement and muscle tone that classify every 30 seconds of the night into a stage. From that, researchers derive numbers like time to fall asleep, time awake after sleep onset, sleep efficiency (the share of time in bed spent asleep) and the minutes spent in each stage.
A 2021 systematic review and meta-analysis pooled these recordings across studies comparing smokers with non-smokers (Catoire et al., 2021, systematic review and meta-analysis). Smokers had less slow-wave sleep, the deep stage most associated with feeling restored, took longer to fall asleep and spent more of the night awake. The direction was consistent across the included studies even where the size of the effect varied.
The first hours of the night take the hit
Two analyses from the Sleep Heart Health Study, a large American cohort, looked at where in the night the difference shows up. Current smokers had lighter sleep than never-smokers, and the difference was most pronounced early in the night (Zhang et al., 2006, Sleep Heart Health Study). A follow-up using EEG power spectral analysis found the same pattern in the brainwave data itself (Zhang et al., 2008, Sleep Heart Health Study EEG analysis).
That timing matters because it lines up with pharmacology. Nicotine's plasma half-life is roughly two hours (Benowitz, Hukkanen & Jacob, 2009), so a cigarette at 11 PM leaves about half its nicotine in circulation at 1 AM and a quarter at 3 AM. The same-night evidence
Most sleep studies compare smokers with non-smokers, which leaves open whether the difference is the person or the cigarette. The Jackson Heart Sleep Study asked a sharper question: on nights when a participant used nicotine within four hours of bedtime, how did that night compare (Spadola et al., 2019, Jackson Heart Sleep Study)?
Evening nicotine was associated with more wake after sleep onset and lower sleep efficiency on that night. The often-quoted 42-minute reduction in sleep duration applied specifically to participants with insomnia symptoms, and the overall effect on duration was not significant, so it is worth being precise: the robust finding is more time awake and less efficient sleep, not necessarily a shorter night for everyone.
It's the nicotine, not only the withdrawal
A reasonable objection is that smokers sleep badly because they go into withdrawal overnight. Withdrawal does disturb sleep (Jaehne et al., 2009, review), but an experiment in non-smokers separates the two: transdermal nicotine, with no smoking and no withdrawal, fragmented sleep in a dose-dependent way (Gillin et al., 1994). Nicotine itself is enough.
The German Multicenter Study on Nicotine Dependence adds that heavier dependence tracks with worse sleep, both subjectively and on the recordings (Cohrs et al., 2014, German Multicenter Study on Nicotine Dependence), and a community survey found people who smoke at night report worse sleep quality (Nuñez et al., 2021).
Why this matters even if you're not quitting
Poor sleep is not just unpleasant. Smokers with insomnia symptoms were less likely to stop smoking in a Swedish cohort (Hägg et al., 2020), which suggests that protecting sleep may be worth doing before any conversation about quitting, not after.
The honest summary: the evidence that smoking near bedtime is associated with worse sleep is consistent across designs, from pooled sleep-lab data to same-night comparisons to a controlled nicotine experiment. What no study can tell you is how much your own sleep would change. The cheapest way to find out is to move the last cigarette earlier and watch what happens.
