How long before bed should you stop smoking?

Short answer: the research looked at four hours, nicotine's half-life is about two, and the practical way to get there is one hour at a time. Here is the arithmetic and the evidence behind each number.

Nicotine's half-life, and what it means for tonight

When you finish a cigarette, nicotine in your blood peaks within minutes and then falls as the liver metabolizes it, mostly into cotinine. The plasma half-life of nicotine is about two hours (Benowitz, Hukkanen & Jacob, 2009; Hukkanen, Jacob & Benowitz, 2005). Half-life is the time for the concentration to halve, so the curve looks like this:

Two caveats. Half-life varies between people: genetics, liver enzyme activity and even menthol can shift it, and heavy smokers accumulate nicotine through the day so the starting level is higher. And cotinine, the metabolite, has a half-life closer to 16 hours, which is why urine and saliva tests detect smoking for days. Cotinine is far less active than nicotine, so it is nicotine, not cotinine, that this guide is about.

Why four hours

The Jackson Heart Sleep Study examined nights on which participants used nicotine within four hours of bedtime (Spadola et al., 2019, Jackson Heart Sleep Study). Those nights showed more wake after sleep onset and lower sleep efficiency than the same participants' other nights. Four hours is two half-lives, the point at which about a quarter of the nicotine remains. It is a window chosen by the researchers, not a physiological cliff, but it is the best-studied cutoff we have.

How to actually get there

If your bedtime is 11 PM, a five-hour window means the last cigarette at 6 PM. Few people can jump there from an 11 PM habit and keep it. The evidence favors a schedule:

  • Start at one hour. Last cigarette by 10 PM. Almost anyone can do this tonight, and it already removes the cigarette with the most nicotine still present at sleep onset.
  • Add an hour each stage. A randomized trial of scheduled smoking found that gradual, scheduled reduction outperformed unscheduled reduction (Cinciripini et al., 1995, randomized trial). The schedule is the point.
  • Decide the time in advance. People who commit to a specific when-and-where follow through far more often than people with a general intention (Gollwitzer & Sheeran, 2006, meta-analysis). "Last cigarette at 9 PM" beats "smoke less in the evening".
  • Expect the first evenings to be uncomfortable. Withdrawal symptoms peak in the first days and fade over weeks (Hughes, 2007). The window only moves an hour at a time, which keeps that manageable.
  • Watch your sleep, not just your cigarettes. Tracking progress toward a goal improves the odds of reaching it (Harkin et al., 2016, meta-analysis). If you wear a watch that records sleep, compare nights.

How long does nicotine stay in your system?

Nicotine's plasma half-life is about two hours, so roughly half is cleared every two hours. After four hours about a quarter remains and after five hours about 18 percent. Its metabolite cotinine lasts far longer, around 16 hours, which is why tests detect smoking for days.

Is four hours before bed enough?

Four hours is the window the Jackson Heart Sleep Study examined: nicotine within four hours of bedtime was associated with more time awake and lower sleep efficiency. Stopping at least four hours before bed keeps you outside that window. Five hours adds a margin and leaves under a fifth of the nicotine circulating.

What if I can't manage four hours?

Start with one. Moving the last cigarette one hour earlier is a change almost anyone can make tonight, and each extra hour clears more nicotine. Gradual, scheduled reduction has better evidence than abrupt unscheduled cuts.
